Abstract: This study examines the relationships among three-dimensional facial proportions, facial satisfaction, and intention to undergo facial aesthetic modification by integrating the Theory of Planned Behavior (TPB) with objective 3D facial measurements. A sequential mixed-methods design combined bibliometric analysis, qualitative interviews, and a quantitative study of Thai women aged 18–40 years (n = 150). Facial proportions were assessed using the ISEMECO 3D D8 Skin Analyzer, followed by the validated FACE-Q and TPB-based questionnaires. Results showed no significant association between 3D facial proportions and facial satisfaction (p > .05). However, within the integrated model, facial satisfaction negatively predicted modification intention (β = -0.128, p = .038), while subjective norm emerged as a significant positive predictor. These findings challenge the role of objective facial morphology in aesthetic decision-making, emphasizing subjective evaluation and social influence. This study provides novel evidence for a perception-driven framework with implications for personalized aesthetic care.
